Pursuant to the Court's Order Granting Plaintiffs' Motion for Preliminary Approval of Class Action Settlement dated December 12, 2025, Class Members who wish to participate in the Settlement shall complete and submit the Proof of Claim and Release ("Proof of Claim") in accordance with the instructions contained therein. To obtain a copy of the Proof of Claim, click here. The deadline to submit a timely Proof of Claim is April 6, 2026.

You may submit your Proof of Claim using any one of the following methods. Each method is described below, with links to the requirements for each submission. Your submitted Proof of Claim will include a certification under penalty of perjury that your Proof of Claim is accurate and complete.

Whichever submission method you choose, you are required to provide documentation to support your claimed transactions. Please remember to provide supporting documentation of all transactions listed on your Proof of Claim. In the event that you submit your Proof of Claim online, you must retain confirmation of such filing.

If you fail to execute and submit a timely, properly completed and addressed Proof of Claim, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.
